When engaged in deployment or stowage of bulk hoses, the

Banks

m

an

and handlers must never stand in a

position where there is potential for entanglement in a hose or being struck by a falling hose. Other

personnel must be kept well clear of the area. The

Banks

m

an

shall

stand in a position where a clear view of

the hose handling operation can be attained.

x

Hoses

shall

be thoroughly checked visually for quality; if there be any doubt as to the hose being fit

for purpose it

shall

be replaced;

x

Never accept hoses before the vessel is stabilised in position under the crane;

x

Never accept hoses that have insufficient flotation collars along its whole length, or insufficient

reflective tape or equivalent (if applicable);

x

Try to send hoses back up to the installation as soon as loading/discharging is completed;

x

The deck crew

shall

never disconnect a hose from the crane-hook before they have secured the

hose on deck;

x

If working more than one hose at the same time, always ensure that all hoses are of approximately

the same length, in order to prevent accidental submersion of a hose below the water line;

x

If the

s for any reason find it necessary to reduce the number of

hoses, give the installation clear reasons for the decision;

x

When discharging dry bulk, endeavor to keep the vessel away from the installation’s ventilation

intakes;

x

Prior to discharging fresh water, lines

shall

be flushed through in order to avoid discharging

contaminated freshwater;

The following actions

shall

be taken in event of hose breakage/emergency break-away:

x

Shutdown all transfers - shutdown the pumps on vessel side and request that the pumps are stopped

on the installation side;

x

Inform the installation - call the installation and inform them as soon as possible of the problem;

x

The earlier the alert is sent, the better the possible consequences can be limited;

x

Stop all crane operations;

x

Try to keep applicable valves open to allow the circuit to be drained by gravity;

x

When possible release the connection on the vessel side while slack is in the hose (tools

shall

be

ready in position);

x

Lay the hose on the deck, clear of any cargo and other obstructions;

x

When possible, send the hose back to installation, if not; the hose

shall

be positioned in order to

allow it to be released over side if the vessel continues to drift;

x

Move away clear of the installation;

x

When the hose is disconnected and clear, move out of the 500 meter zone to prevent any further

incident with the installation.
